编者按：2022年12月10日-13日，第64届美国血液学会（ASH）年会在美国路易斯安那州新奥尔良隆重召开，全球学者齐聚一堂，共同见证血液系统疾病的最新诊疗进展。本次大会期间，Gerald Illerhaus教授关于原发中枢神经系统淋巴瘤(PCNSL)自体干细胞移植与非清髓免疫化疗巩固治疗的比较研究入选大会，《肿瘤瞭望》现场采访了Illerhaus教授，Illerhaus教授介绍了PCNSL的治疗现状、研究细节以及该研究对临床实践的指导作用。

肿瘤瞭望：原发中枢神经系统淋巴瘤(PCNSL)是一类侵袭性极高，预后很差的淋巴瘤，首先请您介绍一下PCNSL治疗的基本现状及前沿的治疗方式？

 

Illerhaus教授：对于PCNSL患者，诱导治疗必需采用尽可能强烈的治疗方案，以达到完全缓解或部分缓解。随后如果符合移植条件，在干细胞移植前给予高剂量化疗，患者预后最好。对于老年患者，情况有所不同，应根据患者状况采用不同的治疗策略。如果患者年龄较大但身体状态较好，也应考虑进行自体干细胞移植，如果身体条件不允许则应考虑其他治疗选择。

 

Dr Illerhaus:We have to go as intensively as possible for patients with induction treatment to achieve a complete or partial response overall.Then，if the patient is eligible for transplantation，we have the best prognosis after high-dose chemotherapy before stem cell transplantation.In elderly patients，it is different.We have to use other strategies.If the patients are fit and old，they are also candidates for autologous stem cell transplantation，and if not，other options should be tested.

 

肿瘤瞭望：您的一项研究入选了ASH大会的LBA，比较了非清髓免疫化疗vs大剂量化疗+自体干细胞移植作为PCNSL巩固治疗的疗效差异，请您详细解读一下?

 

Illerhaus教授：我们的研究表明，诱导治疗后，患者随机接受大剂量化疗和自体干细胞移植或是常规治疗，干细胞移植患者的死亡风险降低54%，疗效优于常规治疗，大比例患者获得了更好的治疗结果。

 

Dr Illerhaus:Our study showed that after induction，randomization between high-dose chemotherapy and autologous stem cell transplantation versus conventional treatment led to a reduction in risk of death of 54%.It is much more effective than conventional treatment and better outcomes were achieved in a substantial proportion of patients.

 

肿瘤瞭望：该研究对于我们临床上改善PCNSL长期生存意义如何?能否有望改变PCNSL治疗实践？

 

Illerhaus教授：我个人认为应该有更多患者考虑进行移植治疗，并进行移植相关评估。诱导治疗对于患者能否获得缓解非常重要。MATRix是德国和其他欧洲国家乃至全世界的标准治疗，许多患者获得很好的缓解，但也必需十分谨慎，因其的确具有一定毒性。如果条件允许，患者应进行高剂量化疗和干细胞移植。

 

Dr Illerhaus:I think more patients should go for transplant，and evaluated for transplant.The induction period is very important to help patients achieve remission.MATRix is the standard-of-care in Germany and other European countries，and worldwide.This leads to good remission in a lot of patients.It is toxic and you have to be careful with the protocol.If it is possible，high-dose chemotherapy and stem cell transplantation should occur.
